You can refer anyone impacted by a recent Parkinson’s diagnosis. We’ll reach out proactively, and securely, to connect them to high quality support.
Who is Parkinson’s Connect for?
We aim to be there for every single person with Parkinson’s and their family. As we roll out and expand the service, we’ll open it to people at all stages of their journey with Parkinson’s.
Right now, as we launch Parkinson’s Connect across the UK, we’re opening part of the service – referrals from healthcare professionals – for recently diagnosed people only. This means anyone who is still coming to terms with their diagnosis and looking for initial information and support. There’s no strict time limit, because we know that getting a Parkinson’s diagnosis affects everyone differently.
We’ve trialled the referral service with 6 healthcare teams in England and Scotland, who have referred over 370 people since January 2020. As we roll out referrals across the UK and more people start using the service, we’ll keep improving and developing referrals and Parkinson’s Connect together based on what we learn along the way.
How to refer your patients into Parkinson’s Connect
Parkinson’s Connect referrals build on your trusted relationship with your patients. Once someone is referred, with consent, we proactively contact them, or their family and friends, and provide tailored support – building on the support you already provide.
We’ve made the process as simple as possible and we’ll support you with any questions along the way. You just need to complete an online form to register your intention to refer patients.
Once we have your details, we’ll get in touch to establish a lead contact in your team and provide them with an Information Sharing Agreement and supporting documents. Once these are complete, you’ll receive your unique referral link and you’ll be ready to refer patients.
We’ll help you throughout the process of getting set up, and you’ll be able to email or call us if you have any questions. If you choose, you can also receive updates and send us feedback to improve the referral process.
