The 4 'A's test (4AT) is a quick, validated, and free bedside screening tool for delirium and cognitive impairment proven effective in people with Parkinson's. The 4AT assesses alertness, awareness, attention, and acute mental state changes. Research in 2025, funded by Parkinson's UK and published in Age and Ageing, confirmed its high efficacy in people with Parkinson’s. 

It takes less than 2 minutes for health professionals to complete, requires no formal training, and is available instantly from the 4AT website, including downloadable and mobile app versions. It is available in over 20 languages.
