The care plan is for patients with Parkinson's living at home with a care package or in a residential/nursing home setting.

It has been developed by Pete Smith at Milton Keynes Primary Care Trust (now NHS Milton Keynes), and updated by Tess Adams, Parkinson's Nurse, Central and North West London NHS Foundation Trust.

It forms Appendix A in the Providing better support to people with Parkinson's in the community and preventing unnecessary hospital admissions interactive resource.
