Congratulations! You've taken the first step to nominating an outstanding health or care professional for an Excellence Network Award.

Rising star

Nominations are welcome for health or care professionals from their colleagues.

Lifetime achievement

Nominations are welcome for health or care professionals from their colleagues, or from people with Parkinson's, their families and friends.
 

People's choice (nurse, doctor and therapist)

Nominations are welcome from people with Parkinson's, their families and friends. You can complete one nomination form for each of the sub-categories of nurse, doctor or therapist.

You can submit nominations until Friday 10 October 2025.

 

Tips for your nominations

Read the nomination form and then gather the information you'll need in a separate document. Copy and paste it across to the form when you're ready.

You'll need to tell us:

  • who the person is and why you're nominating them
  • their contact details
  • where they're based, for example, their city or hospital
  • why they're an example of good practice in delivering care and support to people with Parkinson's. We've listed general judging criteria on the nomination form, so try to demonstrate those in your statement.
