Transforming care for older adults living with frailty
This online conference focuses on the important issue of transforming care for people with frailty.
The event will provide an essential guide to improving the quality of care for people with frailty and supporting them to stay out of hospital. It'll focus on the adherence to the NHS England FRAIL strategy for the development and improvement of acute frailty same day emergency care services, and the National CQUIN (Commissioning for Quality and Innovation).
The redesign of frailty services through the development of neighbourhood health is crucial to meeting increasing demand and improving the quality of life for older people. The conference will update delegates on neighbourhood health and frailty.
The day will also enable delegates to:
- network with colleagues working to improve care for frail older people
- learn from outstanding practice in developing dedicated frailty services
- reflect on national developments and learning
- identify key strategies for preventing unnecessary hospital admissions
- understand the principles and practicalities of setting up and running a virtual frailty ward
- explore priorities and challenges in improving end-of-life care for people with frailty.
