Transforming care for older adults living with frailty

The event will provide an essential guide to improving the quality of care for people with frailty and supporting them to stay out of hospital. It'll focus on the adherence to the NHS England FRAIL strategy for the development and improvement of acute frailty same day emergency care services, and the National CQUIN (Commissioning for Quality and Innovation).

The redesign of frailty services through the development of neighbourhood health is crucial to meeting increasing demand and improving the quality of life for older people. The conference will update delegates on neighbourhood health and frailty.
