Community Awards 2026 Nomination Form

Please complete the form below to nominate someone for a 2026 Community Award.
 
To ensure your nominee receives a Community Award certificate, we'll need their postal address. If you don't have these details handy, we recommend gathering them before you begin. If you can't find this, we recommend entering your own address to pass the certificate on yourself.
 
Please use a separate form for each nomination. 
 
If you'd like to make a group nomination, please provide one postal address where the certificate should be sent. 
 
There's no single winner for the awards, everyone who's nominated will be thanked for their wonderful contributions!
