The Toolkit maps out the care pathway a dementia patient will follow and provides an aide-memoire for the services and support needed from diagnosis to end-of-life care.

Dementia Academy faculty members Dr Iracema Leroi, Sue Thomas, Dr Tony Burch, Dr Ross Dunne and Dr Ross Overshott created the Toolkit with the then Greater Manchester, Lancashire and South Cumbria Strategic Clinical Network (SCN).

In April 2016 this became Greater Manchester and Eastern Cheshire SCN.
