The Parkinson's Self-Assessment Tool is designed for people with Parkinson's to complete over a 2-week period and then share with their nurse, doctor or other healthcare professional.

It's to show how Parkinson's affects people on a day-to-day basis and help develop the best possible treatment plan.

The document aims to improve communication between people with Parkinson's and healthcare professionals.

The tool was developed with The Cure Parkinson's Trust and fully funded by GlaxoSmithKline.
