Anticipatory care plan

This anticipatory care plan outlines common complications people with Parkinson's might experience in the community, why these may arise and what actions professionals can take to address them to prevent an unnecessary hospital admission.

The care plan is for patients with Parkinson's living at home with a care package or in a residential or nursing home setting.

It was developed by Pete Smith at Milton Keynes Primary Care Trust (now NHS Milton Keynes), and updated by Tess Adams, Parkinson's Nurse, Central and North West London NHS Foundation Trust.

It forms Appendix A in the Providing better support to people with Parkinson's in the community and preventing unnecessary hospital admissions interactive resource.
