Hi everyone, I work in the Parkinson's Policy and Campaigns team and am writing a consultation response on unsafe discharge from hospital. The consultation focuses on the fact that often carers are not informed about what is happening which can result in people being discharged in the middle of the night, or sent home with no support. It also talks about patients who are ready to leave hospital, but with no care package organised, their discharge is delayed. I'd love to be able to include some real stories from people who have experienced this. If you have, let me know.
