Delivering for Health
Delivering for Health maps out the Scottish
Executive’s plans to implement the recommendations of the National
Framework for Service Change. Its focus is on shifting towards more
preventative medicine and more continuous care in the
community.
Key features include improving help and
support so that people with long-term conditions can play an
increasing role in managing their condition themselves and treating
carers as partners in the provision of care.
The document sets out how people who need
specialist treatment in hospital will get access to a good, safe
service provided by the right person even if that means they have
to travel, while those who are older, frailer or liable to frequent
hospital admission will get co-ordinated care provided locally.
