Have you sent a Yellow Card report to the MHRA?

The Medicines and Healthcare products Regulatory Agency (MHRA)
has identified a problem with delivery of Yellow Cards, which
patients can use to report adverse drug reactions or side
effects.
Some Yellow Cards have been returned to sender due to an error
at Royal Mail.
If you have posted a Yellow Card report and have either had it
returned or not received an acknowledgement letter, you will need
to resend the report.
This can be done via the Yellow Card Scheme online
reporting site or by sending a new Yellow Card.
About the Yellow Card Scheme
The Yellow Card
Scheme is vital in helping the Medicines and Healthcare
products Regulatory Agency (MHRA) monitor the safety of the
medicines that are on the market.
Patients can send a Yellow Card report to tell the MHRA about
suspected safety issues or side effects of medication.

encourages people affected by Parkinson's to fill out a Yellow Card
report if they experience delays or a missed dose of medication in
hospital.
